The Sankaty ExecSelect™ Plan
A Powerful New Solution for Your Benefits & Recruitment Strategy

Thank you for your interest in our exciting new ExecSelect™ benefit for your Key Employees.  If you would like to complete an application, we will be happy to assist you! 

• Contact us directly using the Contact Us form and one of our representatives will respond promptly.
• Contact your Broker to provide some basic information and assist you with our Employer Proposal; or you may select the Employer Proposal form here to submit to us directly online.  We will contact you to review your application.
• Once your Proposal is completed, we provide you with an agreement for mutual acceptance and invoice you for the Fixed Annual Premium.  We then coordinate the following with you:
  • You will receive a Sankaty Light Benefits Certificate of Insurance. 
  • You will receive a Sankaty Light Benefits ExecSelect™ Group Policy.
• When your company is approved, your designated participants will need to fill out and submit the following forms which will be provided to you by Sankaty:
  • Employee Enrollment Form
  • Your Eligible Employees and dependents will receive a Sankaty Light Benefits Certificate of Insurance.
  • Please see the Employee page for additional details about Privacy, HIPAA and Electronic Funds Transfer (EFT).
